St. Charles Health System is currently seeking an experienced Pharmacist
to join the exceptional Clinic Care Management Team.
Position Summary:
The Clinic Pharmacist is responsible for management of patients in the ambulatory setting through a collaborative model that includes clinic providers, staff, resource specialists and clinical resource coordinators. This includes disease state management, patient education, optimization of drug therapy regimens, and other initiatives designed in collaboration with the clinic providers and staff. This position does not directly supervise any other caregivers. The Clinic Pharmacist will be housed in the Family Care Center in Bend (or alternatively a clinic that will most utilize the position), with periodic and regular visits to the Sisters, Redmond, Prineville (IE all other clinics and eventually the Madras site.
Education:
Graduate of School of Pharmacy required. Advanced degree (Doctor of Pharmacy preferred) with residency training or equivalent work experience required.
Licensure/Certification/Registration:
Valid Oregon driver’s license and ability to meet SCHS driving requirements
Ability to travel to business functions/trainings/meetings and all SCHS worksites required
Experience:
Experience in ambulatory setting with disease state management preferred.
Licensure/Certification:
Current registration by the Oregon Board of Pharmacy required..
Personal Protective Equipment:
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
Experience:
Minimum1 year of pharmacy experience required
Experience in managed care, disease management, corporate wellness and/or health promotion or equivalent desirable

Basic to intermediate ability and experience in computer applications, specifically electronic medical records system and MS Office
Position Specific:
Engages in an environment of continuous learning, embracing change and mastering competencies of new skills
Knowledge of chronic disease management and health promotion, case management, health prevention in counseling situations, government regulations, team process, quality improvement, managerial functions and computer process.
